The Challenges in Integrating Care Coordinators into Cancer Care – an Analysis of Australian Evaluation Data (#241)

In Australia, a range of system, team and health professional led strategies have been implemented to improve cancer care coordination. Over recent years, a number of large scale evaluation reports have been published reporting on impact of the cancer care coordinator role as one strategy to achieve the improved coordination. These evaluation reports have considered various outcomes and processes associated cancer care coordinator positions, including roles and functions, models of care, outcomes and governance arrangements.  The reports include some evidence which supports the value of such roles in improving patient experience in the Australian context.  However a number of common themes emerge relating to challenges and limitations associated with the role, and the significant variation that exists with respect to the role and its impact. Future advances in improving care coordination will require that greater attention is given to identifying and evaluating outcomes from care coordination interventions.
